Work Environment

This position is primarily onsite and involves traveling to patients' residences throughout the Woodcliff Lake area and surrounding communities. You will work independently with clients, yet remain a vital part of our professional team. Our facility ensures that technical support, administrative resources, and equipment are readily accessible to enhance your practice. You will enjoy a structured schedule—Monday through Friday, from 9 AM to 5 PM—allowing for work-life balance while delivering essential therapy services.

Key Responsibilities

• Conduct comprehensive evaluations of patients through interviews, observations, and physical tests to understand their unique physical capabilities and limitations.
• Develop and implement personalized physical therapy plans that align with patient goals and clinical best practices.
• Utilize evidence-based interventions including but not limited to manual therapy, therapeutic exercises, and functional training to support recovery.
• Instruct patients and their families on effective techniques for pain management and highlight exercises that facilitate improvement between therapy sessions.
• Educate patients on ergonomic adjustments, posture improvement, and injury prevention to encourage long-term health and wellness.
• Maintain accurate and meticulous records of all patient evaluations, treatment plans, and outcomes in compliance with state regulations.
• Monitor patient progress regularly and adjust therapeutic plans to reflect ongoing patient assessment; report all relevant changes to the attending physician and family members.

Patient Population / Client Focus

As a Homecare Physical Therapist at Mend Force, you will work with a diverse range of patients, including elderly individuals recovering from surgery, those with chronic pain conditions, and others requiring rehabilitation after injury or illness. Your role will be pivotal in restoring their independence and enhancing their functional capabilities through targeted therapeutic interventions.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited program is required.
• Active Physical Therapy license in New Jersey.
• A minimum of two years of hands-on experience in physical therapy, ideally in a homecare or outpatient setting.
• Expertise in clinical assessment, manual therapy, and rehabilitation strategies.
• Current CPR and First Aid certification is preferred; additional certifications related to geriatrics or neurological rehabilitation will be an asset.
• A valid driver's license and reliable transportation for home visits.

Preferred Experience

Experience in homecare settings or outpatient rehabilitation is highly preferred, as well as familiarity with patient management software that helps track progress and document treatment plans. Candidates who possess exceptional interpersonal skills to engage with patients and families will excel in this role.

Compensation and Benefits

The compensation for this position ranges from $110,000 to $120,000 annually, commensurate with experience and qualifications.
