Key Responsibilities\n

• Conduct thorough assessments of patients to tailor individualized treatment plans that address their specific mental health needs.

• Maintain open communication with clinical supervisors, direct care staff, family members, and guardians regarding patient progress and treatment adjustments.

• Provide a structured schedule to supervisors, detailing appointments, home visits, and observations to monitor treatment advancements.

• Coordinate home visits that comply with clinical standards for in-home assessments, chart evaluations, and staff interviews.

• Oversee the implementation of treatment plans, ensuring they are carried out effectively and consistently.

• Participate actively in Interdisciplinary Team Meetings to discuss patient goals and share insights for collective improvement.

• Document monthly and quarterly progress reports that reflect the ongoing assessment and treatment outcome for each patient.

• Organize and facilitate training sessions for staff to enhance competency in implementing treatment plans and completing required documentation.

• Follow up with ongoing assessments through interviews and observations, ensuring continuous support for patients.

• Maintain accurate and accessible patient records, safeguarding confidentiality and making sure that information is readily available to authorized parties.

Qualifications\n
