Key Responsibilities\n

• Conduct ongoing, personalized assessments to identify and address the unique needs and progress of each patient.

• Communicate effectively with supervisors, fellow counselors, family members, and guardians to keep everyone informed of patient progress.

• Develop and manage a tentative scheduling of appointments and home visits to facilitate continuous support and monitoring of clients.
\n
• Coordinate home visits including necessary observations, chart reviews, and interviews to thoroughly evaluate client needs.
\n
• Ensure that program plans are implemented effectively and adjustments are made as needed based on client feedback and outcomes.
\n
• Engage actively in Interdisciplinary Team Meetings and collaborate on care strategies to enhance patient outcomes.
\n
• Maintain detailed records, such as progress notes and treatment plans, ensuring accessibility for all interested parties.
\n
• Organize and conduct staff training sessions to ensure adherence to best practices and effective implementation of treatment plans.

• Serve as the public liaison between the organization and the mental health community to promote awareness and meet developmental goals.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• Must possess an active and unrestricted license as a Professional Counselor (LPC) in the state of Texas.
