Key Responsibilities\n

• Conduct comprehensive assessments to identify and evaluate patient needs, developing tailored care plans accordingly.

• Facilitate clear communication between supervisors, care staff, families, and other stakeholders regarding patient progress and ongoing treatment strategies.

• Schedule and monitor appointments, home visits, and observations to support effective treatment delivery.

• Perform in-home visits to conduct observations and collect relevant data for comprehensive care evaluations.

• Ensure the effective implementation of treatment plans and monitor their efficacy through ongoing evaluation.

• Participate in Interdisciplinary Team Meetings to collaborate with other healthcare professionals on comprehensive care strategies.

• Document monthly and quarterly progress notes detailing client advancements and areas requiring additional focus.
\n
• Provide training for staff members to ensure all team members are proficient in implementation and documentation processes.

• Conduct follow-up interviews and home visits to ensure consistent support for individual needs.

• Maintain accurate and thorough records to guarantee information accessibility for all relevant parties involved in client care.

• Act as a liaison with the public and broader mental health community to advocate for client development and program enhancement.

• Participate in the development of training materials aimed at improving program efficacy for individuals served.

Qualifications\n
