Key Responsibilities\n

• Conduct comprehensive assessments of clients through phone or in-person interviews, evaluating needs, strengths, and areas for improvement.

• Develop tailored support plans, coordinating with clients, families, and other professionals to set measurable goals and interventions.

• Monitor clients' progress toward their goals, making necessary adjustments to treatment plans based on evidence and client feedback.

• Provide crisis intervention services, ensuring clients feel safe and supported during challenging moments.

• Facilitate referrals to community resources, ensuring clients receive the holistic care they need for recovery.

• Maintain detailed and accurate records of client interactions, treatment plans, and progress notes in our secure electronic health record system.

• Participate in case conferences and collaborate effectively with interdisciplinary teams to ensure integrated service delivery.

• Act as a key-worker for assigned clients, serving as their advocate and primary contact throughout the care process.

Compensation and Benefits\n

The annual salary for this position ranges from USD 75,000 to 90,000, commensurate with experience and qualifications.
