Work Environment

This role is primarily onsite, where you will travel directly to patients' homes in Sarasota and the surrounding areas. Key Responsibilities

• Conduct thorough assessments of patients' physical conditions, reviewing medical histories and current physical capabilities.
• Develop, implement, and modify individualized treatment plans focusing on outpatient orthopedics, particularly in areas related to spine surgery recovery.
• Engage in effective communication and collaboration with patients, their families, and other healthcare professionals to ensure comprehensive care.
• Document patient progress, treatment outcomes, and modifications to care plans consistently, using electronic health record (EHR) systems.
• Maintain professional integrity and comply with all healthcare regulations, laws, and guidelines applicable to home care settings.
• Educate patients and families on exercises and strategies to improve mobility and manage pain effectively.
• Participate in team meetings and training sessions to improve departmental policies, enhance service delivery, and stay updated with best practices.
• Promote a safe environment by demonstrating and enforcing proper body mechanics during therapy sessions.
• Actively contribute to community health projects and initiatives that promote wellness and prevention.

Patient Population / Client Focus

You will work primarily with individuals recovering from surgical procedures, particularly spinal surgeries, as well as those dealing with chronic pain, orthopedic issues, or post-injury rehabilitation. Our patient population ranges from elderly clients needing assistance with mobility to younger patients looking to regain full function following an injury. A compassionate approach is essential to foster trust and motivation in your patients.

Qualifications

• Graduate degree from an accredited physical therapy program (BSPT, MSPT, or DPT) with a focus on practical application in home care settings.
• Current licensure as a Physical Therapist in the state of Florida, with the obligation to maintain an active, good standing license for the duration of employment.
• CPR certification is required upon hire or must be obtained during orientation, with a commitment to keep certification current.

Preferred Experience

• Experience in outpatient orthopedics and treating patients recovering from spine surgery.
• Familiarity with home care practices and the specific needs associated with delivering therapy in a home environment.
• Proficiency in using Microsoft Office applications (Word, Excel, and email) for documentation and communication.
• Knowledge of electronic health record (EHR) systems is a plus.

Compensation and Benefits
