Key Responsibilities\n

• Engage proactively with participants to identify and implement strategies that minimize the need for crisis services.

• Complete and maintain accurate crisis alerts while coordinating care with county crisis services for timely interventions.

• Manage transitions for clients requiring hospital or inpatient services, ensuring comprehensive communication with hospital staff regarding discharge plans and follow-up care.

• Document client interactions meticulously in our electronic health record (EHR) system, maintaining compliance with clinic policies and best practices.

• Actively participate in weekly team meetings to coordinate care and share insights on program participants, fostering a collaborative environment.

Patient Population / Client Focus\n

Your primary focus will be on a diverse population of clients experiencing a wide range of psychiatric, social, and emotional challenges. As a professional counselor, you will work with individuals, couples, and families, tailoring your approach to meet their cultural, linguistic, and psychological needs. Qualifications\n

• A Master's degree in Counseling, Social Work, Marriage and Family Therapy, or Mental Health Counseling from an accredited institution.

• Active and valid licensure as a Marriage and Family Therapist (LMFT), Licensed Professional Counselor (LPC), or Licensed Clinical Social Worker (LCSW).

• Proven ability to build rapport quickly with clients from varied backgrounds.

• Excellent interpersonal skills with a focus on culturally sensitive and appropriate communication.

Compensation and Benefits\n
