Work Environment

This position entails providing onsite physical therapy services within the clients' homes throughout Punta Gorda and its surrounding areas. The home care setting allows us to deliver customized treatment in a comfortable and familiar environment, which can significantly aid in the rehabilitation process. Our team collaborates closely with healthcare professionals, ensuring that all aspects of patient care are seamlessly integrated to achieve optimal outcomes.

Key Responsibilities

• Direct and manage physical therapy treatment plans focused on preventing disability, alleviating pain, and restoring muscle function while adhering to individualized care plans.
• Supervise and guide Physical Therapy Assistants in effectively implementing prescribed therapy interventions.
• Conduct comprehensive evaluations of clients' physical capabilities, utilizing diagnostic assessments to measure muscle and joint function, and inform treatment adjustments as necessary.
• Assess and modify the home environment for rehabilitation, providing recommendations to enhance patient independence.
• Administer therapeutic exercises, both manual and non-manual, while motivating patients to engage in their rehabilitative exercises actively.
• Implement various physical therapy modalities, including traction and massage, and utilize physical agents as part of treatment plans.
• Evaluate, fit, and recommend adjustments for prosthetic and orthotic devices in collaboration with the healthcare team.
• Participate in interdisciplinary team meetings to discuss client care plans and progress updates, ensuring a holistic approach to treatment.
• Document patient progress thoroughly through clinical records, including initial evaluations, care plans, progress notes, and discharge summaries, in accordance with regulatory requirements.
• Collaborate with patients and their families to educate them on managing and optimizing their therapy routines, including the usage of assistive devices.
• Maintain timely submission of all necessary documentation and orders, following branch policies.
• Conduct ongoing functional assessments and tailor care plans in response to patient progress and needs.

Patient Population / Client Focus

The primary focus of this role will be on orthopedic cases, which may include individuals recovering from surgical procedures, sports injuries, age-related mobility issues, and chronic pain conditions. You will work with a diverse clientele ranging from pediatric to geriatric populations, adapting your approach to cater to each individual's unique needs and ensuring they receive optimal care tailored to their specific rehabilitation goals.

Qualifications

To be successful in this role, candidates must hold a Bachelor's degree in Physical Therapy or a related field, along with a valid state license to practice as a physical therapist. A minimum of one year of relevant experience in physical therapy—ideally in a home care or outpatient setting—is required. Excellent communication and interpersonal skills are essential, as you will be engaging closely with patients and their families to facilitate their recovery journey.

Preferred Experience

While one year of experience is required, preference will be given to candidates with prior experience in orthopedic rehabilitation and those who have worked in home healthcare settings. Familiarity with rehabilitative technologies and outcome measurement tools will be considered advantageous.

Compensation and Benefits

This full-time position offers a competitive annual salary ranging from USD 100,000 to 120,000, commensurate with experience and qualifications.
