Key Responsibilities\n

• Conduct comprehensive assessments to evaluate the emotional, mental, and social health of patients and determine their needs for support. This includes utilizing diagnostic tools, such as the DSM-5, to inform individualized care plans.

• Design, implement, and continuously assess treatment plans tailored to each patient's unique circumstances and cultural background, ensuring that objectives align with the patient's desired outcomes.

• Document and maintain accurate clinical records, treatment progress, and correspondence with regulatory bodies in a timely manner, ensuring compliance with both internal policies and state regulations.

• Facilitate access to necessary resources and referral services, acting as a bridge between patients and other healthcare providers to ensure continuity of care.
