Key Responsibilities\n

As a Licensed Clinical Professional Counselor, your primary responsibilities will include:

• Providing a tentative schedule to supervisors for appointments, home visits, and regular observations, helping facilitate the monitoring and enhancement of patient outcomes.

• Coordinating home visits that include in-home observations, chart reviews, and interviews, ensuring that comprehensive support is provided to clients in their personal environments.

• Ensuring that all program plans are executed effectively, aligning with patient needs and best practices to promote positive health outcomes.

• Engaging in interdisciplinary team meetings to collaborate on and strategize treatment plans, drawing from a diverse array of professional perspectives.

• Completing monthly and quarterly progress notes to track patient development and program efficacy, thereby contributing to organized care documentation.

• Training and certifying staff and team members on the implementation of program plans, ensuring comprehensive understanding and adherence to documentation standards.

• Conducting follow-up interviews and continuing home visits, enhancing ongoing support for individual client needs to maximize therapeutic progress.
