Work Environment

This position is primarily onsite at an assisted living facility, where you will interact closely with residents and collaborate with a multidisciplinary team of healthcare professionals. This environment allows for a unique blend of clinical practice and community engagement, making every workday impactful. Your typical schedule will be from 9:00 a.m. to 6:00 p.m., with some evening clinics until 8:00 p.m. Participation in community outreach events, health fairs, and weekend initiatives may also be required to ensure comprehensive healthcare delivery.

Key Responsibilities

• Conduct thorough health assessments to evaluate the medical needs and conditions of residents.
• Diagnose and manage acute and chronic medical challenges, providing preventive care and health promotion tailored to individual needs.
• Collaborate effectively with local community organizations and stakeholders to develop and implement targeted health education programs.
• Deliver culturally sensitive care and advocate for patient rights, ensuring that residents have access to necessary healthcare services.
• Coordinate referrals to specialists and follow up on care plans to ensure continuity of care across the healthcare continuum.
• Maintain meticulous and compliant medical records, ensuring accuracy in documentation according to healthcare standards.
• Engage in community outreach, representing the organization at health fairs and community events, aiming to enhance health literacy and awareness.
• Stay well-informed about current community health trends and adapting practices according to evidence-based guidelines.

Patient Population / Client Focus

The Nurse Practitioner will mainly work with residents in an assisted living facility, many of whom may be experiencing complex health needs due to age-related conditions or chronic illnesses. You will play a crucial role in fostering an environment that supports patient-centered care and promotes healthy aging, focusing on the unique requirements of each individual.

Qualifications

• A current Nurse Practitioner certification, with an active state nursing license.
• A Master's degree in nursing from an accredited program.
• A minimum of 3 years of clinical experience, ideally with a background in community health or public health settings.
• Exceptional interpersonal and communication skills, enabling you to build meaningful connections with diverse patient populations.
• A dedication to social justice and health equity, advocating for vulnerable communities.

Preferred Experience

• Experience in geriatric care or working within assisted living or skilled nursing facilities is preferred.

Compensation and Benefits

We offer a competitive salary range of USD 120,000 to 130,000 per year, commensurate with your experience and qualifications.
