Work Environment

The Family Nurse Practitioner will work onsite at an Assisted Living Facility within a supportive and collaborative healthcare environment. You will interact with diverse populations and work closely with other healthcare providers, including physicians, nurses, and specialists. Key Responsibilities

• Conduct comprehensive health assessments and screenings for patients, developing personalized care plans that address their unique health needs.
• Diagnose and manage both acute and chronic medical conditions while promoting preventive care and health education.
• Collaborate with local community organizations to create and implement health education programs tailored to the socio-economic characteristics of the population served.
• Advocate for culturally sensitive care and protect patients' rights to access healthcare services seamlessly.
• Coordinate referrals and follow-up care to ensure continuity, establishing strong partnerships with specialists and community resources.
• Maintain meticulous medical records in compliance with HIPAA and other regulatory standards, leveraging electronic health record systems effectively.
• Participate in community outreach events and health fairs, providing educational resources and screenings that empower community members.
• Stay abreast of community health trends and emerging best practices to continuously refine care strategies.

Patient Population / Client Focus

In this role, you will primarily serve patients in an assisted living context, which includes a diverse demographic with varied medical needs and backgrounds. This population often includes older adults who require specialized care and support for age-related health issues, as well as individuals managing chronic conditions. Your ability to provide targeted care while addressing individual concerns will be crucial in enhancing their quality of life.

Qualifications

• Current certification as a Nurse Practitioner with an active state nursing license.
• A Master's degree in nursing from an accredited program is required.
• A commitment to continuing education and staying up-to-date with the latest advancements in healthcare.

Preferred Experience

• 3-5 years of clinical experience in community health or public health settings, demonstrating effective practice in managing complex healthcare needs.

Compensation and Benefits

We offer a competitive annual salary range of USD 180,000 – 200,000, commensurate with your experience and qualifications.
