Key Responsibilities\n

• Conduct comprehensive evaluations and assessments of children and their families, focusing on their psychosocial needs and developmental milestones.

• Develop and implement Individualized Family Service Plans (IFSP) in collaboration with a multidisciplinary team, ensuring tailored support for each unique case.

• Monitor patient and family outcomes based on the effectiveness of the interventions and adjust services as needed.

• Provide ongoing support and education to families, delivering information and skills to enhance their child's development and well-being.

• Utilize evidence-based practices and Natural Learning Environment Practices to develop joint plans with families and recommend strategies for implementation.

• Engage in regular communication with other healthcare professionals and family members to foster a collaborative and supportive environment.
\n
• Ensure compliance with all federal and state regulations surrounding clinical social work practices.
\n
• Adopt a coaching model in your interactions, supporting families within their natural environments.

• Actively participate in team meetings and training opportunities to enhance professional development and best practices.

Patient Population / Client Focus\n

The primary focus of this role is on children from birth to age three who exhibit disabilities or developmental delays. You will be instrumental in providing early intervention services, working closely with families to navigate the complexities of caregiving for young children with unique needs. Your expertise will not only facilitate the children's developmental progress but also empower families through education and support.

Qualifications\n

• A Master’s Degree or Doctorate in Social Work.

• Current licensure by the Department of Public Health as a Licensed Clinical Social Worker (LCSW) is required.
