Work Environment

This role offers an onsite work environment, primarily focusing on home care settings where you will visit patients in their residences. You will have the autonomy to design your schedule, with core working hours from Monday to Friday. Key Responsibilities

• Evaluate and treat patients recovering from injuries, illnesses, or surgeries within their own homes, ensuring a comfortable and supportive atmosphere for rehabilitation.
• Develop and implement individualized treatment plans utilizing advanced therapeutic techniques—including therapeutic exercises, manual therapy, and the appropriate use of specialized equipment—to alleviate pain and enhance mobility.
• Continuously track and document patient progress, treatment responses, and outcomes through regular evaluations, adjusting treatment plans as necessary to meet evolving patient needs.
• Educate patients and their families on therapeutic exercises to promote recovery, mobility enhancement, and safe use of assistive devices, while fostering patient engagement in their own care.
• Participate in ongoing professional development by attending workshops and keeping current with the latest research and best practices in physical therapy.

Patient Population / Client Focus

As a Physical Therapist at Deaconess, you will work with a diverse patient population that includes children, adults, and elderly individuals dealing with various health challenges. This role allows you to make a meaningful impact on patients' lives by helping them regain functional movement, rebuild strength, and enhance their overall quality of life in the comfort of their homes.

Qualifications

• Bachelor's Degree in Physical Therapy; Doctorate (DPT) is highly preferred.
• Current and valid Physical Therapy license in the state of Florida is required.
• Excellent verbal and written communication skills to effectively connect with patients, families, and multidisciplinary healthcare teams.
• Proficient in using electronic health records (EHR) and basic computer applications for documentation and communication purposes.
• Ability to adapt and respond to the dynamic needs of patients in a home care environment.

Preferred Experience

• At least 2 years of professional experience as a Physical Therapist working in rehabilitation settings.
• Previous experience in home health care will be considered an asset but is not mandatory.

Compensation and Benefits
