Key Responsibilities\n

• Conduct comprehensive counseling and psychotherapy sessions, applying individualized treatment strategies that reflect clients' unique experiences and challenges.

• Perform thorough mental status examinations, identifying and responding to changes in clients’ mental health conditions with appropriate interventions.

• Maintain detailed and accurate case notes, progress reports, and treatment plans to ensure continuity of care and compliance with hospice guidelines.

• Demonstrate a calm and empathetic demeanor, effectively managing high-stress situations to provide support and de-escalate client distress.

• Collaborate closely with a multidisciplinary team, including medical doctors, psychiatrists, and case managers, to coordinate holistic care for clients, ensuring all aspects of their health are addressed.

• Facilitate individual and group therapy sessions, adapting therapeutic approaches to meet varying client needs and enhancing community support.

Patient Population / Client Focus\n

This role primarily focuses on clients receiving hospice care—individuals and families navigating the complex emotions surrounding end-of-life issues, including grief, loss, and adjustments to terminal illness. Our clients often face a range of psychological challenges, and your empathetic support will be crucial in helping them navigate these sensitive times with dignity and understanding.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current and valid LCSW license enabling independent practice in the state of Georgia.

Compensation and Benefits\n
