Key Responsibilities

• Conduct detailed psychosocial assessments through various methods such as structured interviews, direct observations, and comprehensive medical record reviews to identify emotional, social, and environmental factors impacting patients’ recovery.
• Provide individual and family counseling aimed at addressing emotional well-being, adjustment issues, and support in coping with life changes following rehabilitation.
• Collaborate effectively with an interdisciplinary rehabilitation team to develop and implement personalized treatment plans that are tailored to each patient's psychosocial needs.
• Assist patients in navigating community resources, including support groups, vocational rehabilitation services, and housing assistance programs, ensuring they have access to the support systems they need.
• Deliver educational resources and training to patients and their families, offering insights into post-rehabilitation services and community support, thereby facilitating smoother transitions into home or alternative care settings.
• Participate actively in discharge planning, ensuring that comprehensive support is maintained as patients prepare to reintegrate into their communities.

Patient Population / Client Focus

As a Licensed Clinical Social Worker at Equis Care Psychological Services, you will serve a diverse clientele, including patients recovering from strokes, spinal cord injuries, orthopedic surgeries, and various neurological disorders.
