Work Environment

This role is performed onsite within our Home Care facility, where you will work directly with patients across various demographics and conditions. Key Responsibilities

• Conduct thorough evaluations by reviewing patient history, implementing assessments, and documenting outcomes in compliance with industry standards.
• Discuss evaluation results and treatment plans with patients and their families, ensuring they are actively involved in the goal-setting process.
• Implement treatment protocols based on individual patient evaluations and adjust as necessary to meet their evolving health care needs.
• Collaborate with a multidisciplinary team, including physicians and occupational therapists, to optimize patient care and address changing conditions that may necessitate plan modifications.
• Maintain meticulous documentation of patient care activities, outlining evaluations, plans of care, treatment progress, and discharge summaries to ensure clarity and continuity of care.
• Effectively manage time while meeting productivity expectations and maintaining an organized schedule.
• Participate actively in community outreach and marketing initiatives to enhance service access and promote health education.
• Engage in ongoing professional development and serve as a clinical instructor, guiding students through their practical training experiences.
• Promote a culture of exceptional customer experience, demonstrating flexibility and adaptability in patient interactions.

Patient Population / Client Focus

Our physical therapists work with a diverse range of patient populations, including those recovering from surgery, managing chronic pain conditions, and seeking rehabilitation after injuries. Qualifications

• A Bachelor's degree in Physical Therapy is required.
• A Doctorate in Physical Therapy is preferred.
• A valid state license to practice physical therapy in California.
• Current certification in BLS for Healthcare Providers by the American Heart Association.
• No prior work experience is required, making this an ideal position for new graduates eager to make their mark in physical therapy.

Preferred Experience

While previous experience is not mandatory, candidates with internships or clinical rotations in outpatient or home health settings will be given preference. Compensation and Benefits

The annual salary for this position ranges from USD 90,000 to 110,000, complemented by a comprehensive benefits package that includes:
