Work Environment

The Physical Therapist will be an integral part of our multidisciplinary team, working onsite in our state-of-the-art rehabilitation facility located in the vibrant community of Immokalee, Florida. Key Responsibilities

• Direct physical therapy treatment aimed at preventing disabilities, relieving pain, and enhancing muscle function based on individualized care plans.
• Oversee and mentor physical therapy assistants, ensuring fidelity to the prescribed plan of care.
• Conduct thorough evaluations of clients using diagnostic assessments of muscle, nerve, joint, and functional capabilities.
• Assess the client's home environment to make recommendations that promote independence and safety during rehabilitation.
• Administer hands-on therapeutic exercises and guide clients through non-manual exercises tailored to their therapeutic needs.
• Utilize modalities such as traction and massage, along with other physical agents, to enhance therapeutic outcomes.
• Evaluate, fit, and recommend modifications for prosthetic and orthotic devices in collaboration with the healthcare team.
• Participate in the continual development and refinement of care plans based on formal evaluations and patient progress.
• Record, track, and report changes in client conditions, communicating effectively with physicians and case managers to adjust care plans as necessary.
• Prepare and maintain accurate clinical documentation, including initial evaluations, progress notes, and discharge summaries, ensuring compliance with OASIS requirements.
• Engage actively in interdisciplinary team meetings to review client cases and contribute to holistic treatment plans.
• Educate patients and families on the use of therapy devices and strategies for maximizing engagement in their recovery process.
• Provide supervision to physical therapy assistants, promoting effective care that aligns with state regulations.
• Complete on-site functional assessments and contribute to community resource evaluations to facilitate patient needs.
• Other duties include maintaining compliance with company policies and participating in quality improvement initiatives.

Qualifications

• Proficient in manual therapy techniques and evidence-based practices relevant to current physical therapy standards.
• Competency in using Electronic Medical Record (EMR) software, particularly Web PT, to maintain thorough and accurate patient documentation.

Preferred Experience

• Experience working in a rehabilitation setting or in-home healthcare is highly desirable.

Compensation and Benefits
