Work Environment

The position is based in our outpatient office in Fairfax, where you will be working directly with patients in a state-of-the-art facility designed for rehabilitation. The clinic is equipped with cutting-edge technology and resources to facilitate effective therapy sessions. You will be part of a multidisciplinary team that includes physicians, occupational therapists, and nursing staff to provide comprehensive care. Work hours are from 8:00 AM to 5:00 PM, Monday through Friday, ensuring a balanced professional schedule.

Key Responsibilities

As a Physical Therapist at Trails Rehabilitation & Wellness Pro, you will be responsible for:

• Understanding and adhering to state practice acts to ensure compliance with laws governing the practice of physical therapy.
• Conducting thorough evaluations of patients to assess their functional limitations and disabilities, and developing tailored treatment plans based on findings.
• Implementing treatment protocols effectively while considering individual patient needs and the dynamic healthcare environment.
• Documenting patient care activities meticulously, including initial assessments, daily encounter notes, interim progress summaries, and discharge evaluations, to provide a coherent narrative of care.
• Collaborating with healthcare professionals and referring patients when necessary, while timely addressing and documenting any changes in patient conditions.
• Participating in discharge planning, ensuring comprehensive follow-up care is outlined for patients post-therapy.
• Engaging actively in team meetings and contributing to initiatives aimed at improving access to care and removing barriers within the healthcare system.
• Maintaining compliance with corporate and clinical service training requirements and state/federal regulations.

Patient Population / Client Focus

The primary patient population for this role includes individuals seeking rehabilitation for orthopedic conditions. This may include post-surgical patients, athletes recovering from injuries, and individuals with chronic pain or mobility issues. Your ability to evaluate patient needs and implement effective therapy will directly impact their journey towards recovery and improved quality of life.

Qualifications

To be considered for this position, applicants must have:

• A Doctorate of Physical Therapy (DPT) from an accredited physical therapy program.
• Current licensure as a Physical Therapist (PT) by the state's Physical Therapy Examining Committee.
• At least two years of relevant experience in outpatient orthopedic settings.
• A valid CPR certification.

Preferred Experience

While not mandatory, candidates with experience in specialized orthotic practices, sports medicine, or specific rehabilitation protocols will be favored. Familiarity with electronic medical record (EMR) systems, and proven success in developing patient-centered treatment plans are also beneficial attributes.

Compensation and Benefits
