Work Environment\n

This position operates in a remote work setting facilitated through virtual platforms that allow real-time communication and collaboration with team members and clients. While the bulk of your work will be performed remotely, you will also engage with clients in their homes or community settings, ensuring direct access to the support they need. The standard work hours are Monday to Friday from 8:00 AM to 4:30 PM, with occasional adjustments needed for client visits or urgent support cases.

Key Responsibilities\n

• Conduct comprehensive assessments to identify the psychosocial needs of seniors, evaluating their emotional, mental, and social wellness.

• Develop individualized care plans in collaboration with seniors and their families, considering their specific goals, preferences, and challenges.

• Provide counseling sessions focused on helping clients navigate significant life transitions such as health changes and loss, ensuring emotional support is readily available.

• Act as an advocate for clients, facilitating access to available community resources and services to help meet their needs effectively.

• Work in conjunction with an interdisciplinary team of healthcare providers, including nurses and caregivers, coordinating efforts to enhance care and support systems.

• Perform regular check-ins and home visits to monitor clients' living conditions, assess safety, and evaluate changes in their psychosocial status.

• Maintain thorough and accurate case documentation including assessment notes, progress updates, and care plan revisions while ensuring confidentiality and compliance with ethical standards.

Qualifications\n
