Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through interviews, observational methods, and an analysis of medical records to determine the emotional, social, and environmental factors affecting patient recovery.

• Provide emotional support and counseling to patients and their families, guiding them through adjustment issues related to health struggles and disabilities.

• Collaborate closely with the multidisciplinary rehabilitation team to develop individualized treatment plans that incorporate patients’ psychosocial needs into their overall recovery strategy.

• Assist patients in accessing necessary community resources, such as support groups, vocational rehabilitation services, and housing assistance, to ensure holistic care beyond the clinic.

• Educate patients and families about available community resources and post-rehabilitation services, facilitating smooth transitions from rehabilitation to independent living.

• Participate in discharge planning discussions to ensure patients are effectively prepared for their next steps, whether transitioning home or to alternative care settings.

Patient Population / Client Focus\n

Our patient population is diverse, including individuals recovering from strokes, spinal cord injuries, orthopedic surgeries, and various neurological disorders. Qualifications\n

• A Master's degree in Social Work from an accredited program.

Compensation and Benefits\n
