Key Responsibilities\n

• Maintain updated knowledge of governmental and private payer networks to ensure smooth transitions for patients accessing various services.

• Engage in collaborative efforts with community and healthcare resources to coordinate comprehensive care tailored to individual patient needs.

• Serve as an advocate for patients and their families, upholding patient rights and accessing protective services as necessary.
\n
• Demonstrate a keen awareness of ethical decision-making and provide advocacy to promote patient welfare.
\n
• Report any suspected abuse or vulnerabilities in accordance with legal mandates to ensure patient safety.
\n
• Participate in processes aimed at preventing hospital readmissions through effective planning and intervention strategies.
\n
• Plan and contribute to transition conferences with patients and families, focusing on their specific needs.
\n
• Utilize technological tools to identify patients at risk of readmission, and develop strategies to mitigate these risks.
\n
• Ensure thorough clinical handoffs for at-risk patients, which may include referrals to appropriate community resources.
\n
• Document and communicate patient progress using electronic medical records to support the treatment plan.
\n
• Provide ongoing support and counseling to patients and their families regarding mental health concerns and treatment options.
\n
• Assist patients with financial or social needs by providing relevant information and resources.
\n
• Collaborate continuously with an interdisciplinary team to anticipate patient needs and adjust care plans as necessary.

• Perform other relevant duties as assigned to support the overall mission of our organization.

Patient Population / Client Focus\n

This role focuses on a diverse population, including individuals facing acute and chronic mental health challenges. Patients may include children, adolescents, adults, and families grappling with various mental health issues, from anxiety and depression to severe behavioral conditions. Qualifications\n

• A Master's or Doctoral degree in clinical psychology, counseling, or social work from an accredited program (e.g., MA, MS, MSW, MFT, MHC, PhD, PsyD).

• Demonstrated ability to efficiently manage a large caseload while providing high-quality care to patients.

Compensation and Benefits\n
