Key Responsibilities\n

• Conduct comprehensive assessments to identify the needs of clients at various stages of their mental health journey.

• Facilitate effective communication among patients, family members, healthcare providers, and interdisciplinary teams to discuss client progress and treatment plans.

• Create and maintain a detailed schedule of client appointments, home visits, and evaluations to monitor progress and outcomes.

• Coordinate in-home observations and chart reviews, ensuring compliance with treatment goals and clinical standards.

• Develop and implement program plans tailored to individual clients, regularly assessing their effectiveness and making necessary adjustments.

• Engage in Interdisciplinary Team Meetings to share insights and collaborate on comprehensive care strategies.

• Document ongoing client progress through monthly and quarterly reports, maintaining compliance with ethical standards.
\n
• Organize and lead training sessions for staff to ensure proficiency in program implementations and documentation requirements.
\n
• Provide follow-up interviews and support to clients, reinforcing their progress and addressing any emerging needs.

• Act as a liaison between the practice and the broader mental health community to advocate for client needs and program development.

Patient Population / Client Focus\n

The Licensed Professional Counselor will work with a diverse client population, including both adults and youth facing a range of mental health challenges. This may include individuals dealing with anxiety, depression, trauma, and substance misuse. Your expertise in family therapy or trauma-informed care will directly enhance the quality of support and engagement these clients receive.

Qualifications\n

• Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• Current and unrestricted licensure as a Professional Counselor (LPC) in the state of Arizona.

• A minimum of two years of counseling experience, preferably within a clinical setting, is mandatory.

• A strong foundation in legal and ethical standards governing mental health practice.

• Exceptional interpersonal and communication skills, with an ability to develop rapport with a variety of clients.
