Full-Time Outpatient Physical Therapist at Rejuvenate Rehab Studio

Position Overview

Rejuvenate Rehab Studio is seeking dedicated and compassionate Physical Therapists to join our dynamic team in San Jose, California. This full-time role will allow you to make a significant impact in the lives of our patients by providing high-quality, personalized physical therapy services in an outpatient setting. Work Environment

You will work onsite at our outpatient facility in San Jose, where you'll find a supportive and collaborative environment. Our team of healthcare professionals is committed to a culture of excellence and teamwork, allowing us to offer the best therapeutic interventions. We strive to maintain a manageable patient load to ensure each client receives the time and attention they deserve. Regular business hours are Monday through Friday, from 8 AM to 3 PM, ensuring a healthy work-life balance.

Key Responsibilities

• Conduct comprehensive physical therapy evaluations for children and adults, assessing physical function and setting patient-centered goals.
• Develop and implement individualized treatment plans that incorporate evidence-based practices and achieve the desired functional outcomes.
• Continuously monitor and reassess clinical signs to evaluate treatment effectiveness, adjust goals, and modify interventions accordingly.
• Maintain accurate and timely documentation through our electronic record system, including SOAP notes, progress reports, and care plans.
• Provide education to patients and their families about the rehabilitation process, addressing challenges and expectations to empower them throughout their journey.
• Achieve minimum utilization and business objectives while advocating for best practices and patient satisfaction.
• Collaborate with colleagues across disciplines at Ensemble Therapy to ensure comprehensive care for shared clients.
• Promote a positive workplace culture and embody the core values of Rejuvenate Rehab Studio in every interaction.

Patient Population / Client Focus

Our patients come from diverse backgrounds, encompassing a wide range of ages and diagnoses. You will have the opportunity to work with children facing developmental challenges, adults recovering from surgeries, and elderly clients seeking to maintain independence through rehabilitation. This role requires sensitivity and adaptability to meet the unique needs of each individual, fostering an inclusive attitude that celebrates diversity.

Qualifications

• A Master's degree or Doctor of Physical Therapy (DPT) from an accredited program is mandatory.
• Active and unrestricted physical therapy license in the state of California.
• A commitment to delivering both in-person and telehealth evaluations and treatments when clinically appropriate.
• A passion for working with patients of differing ages, races, and ethnicities in a multicultural environment.
• Flexibility to adapt to the dynamic healthcare landscape, responding to changes with agility and resilience.
• Excellent collaboration skills, with a focus on customer service and patient satisfaction.

Preferred Experience

While we welcome candidates at all experience levels, previous experience in outpatient rehabilitation settings is a plus. Experience working with children and individuals with specific conditions will give you an advantage in this role.

Compensation and Benefits
