Key Responsibilities\n

• Conduct comprehensive assessments and ongoing evaluations to tailor services to the unique needs of clients, ensuring they receive the support necessary for their mental health journey.

• Actively communicate with supervisors, family members, and other relevant informants to facilitate shared understanding of client progress and treatment strategies.
\n
• Create and maintain a structured schedule of sessions, home visits, and necessary observational activities to keep the treatment plan on track.
\n
• Coordinate essential home visits that comply with requirements for in-home observations, chart reviews, and staff interviews to gather critical data for holistic assessment.

• Ensure adherence to program plans, evaluating effectiveness and making adjustments where necessary to enhance client outcomes.
\n
• Take part in Interdisciplinary Team Meetings and collaborate with other mental health professionals to discuss case management and share insights.
\n
• Generate detailed monthly and quarterly progress notes that document client improvements and delineate goals.
\n
• Facilitate training sessions to certify staff on procedures for implementing program plans and documentation practices accurately.
\n
• Supply ongoing support through follow-up interviews and additional observations, reinforcing clients' needs as they evolve.
\n
• Maintain comprehensive records to guarantee that critical information is available to clients and care teams consistently.

• Serve as a point of contact for the public and mental health community, enhancing our center's connections through community engagement and program development efforts.

Patient Population / Client Focus\n

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• An active and unrestricted Professional Counselor (LPC) license in the state of Texas is essential.

• A minimum of two years of counseling experience, preferably within a clinical setting, is mandatory.
