Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for clients, leveraging diagnostic tools to understand their mental health concerns, strengths, and available social support systems.

• Deliver therapeutic interventions through individual, family, and group therapy sessions, employing evidence-based practices such as cognitive-behavioral therapy and solution-focused therapy.

• Develop, implement, and regularly review personalized treatment plans that are tailored to the unique needs of each client while setting realistic goals and identifying necessary interventions.

• Engage in interdisciplinary collaboration with psychiatrists, psychologists, and other healthcare professionals to ensure a holistic approach to client care.

• Provide crisis intervention when necessary, including conducting risk assessments and developing safety plans to support at-risk clients.

• Document client interactions and treatment outcomes accurately and in a timely manner, adhering to ethical and legal documentation standards.

• Facilitate group therapy sessions, psychoeducational workshops, and support groups to foster community and shared healing among participants.

The primary focus of this role involves working with individuals and families in hospice care who are navigating complex emotional and psychological challenges. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.
