Key Responsibilities

• Conduct thorough evaluations of patients, which include reviewing medical histories, performing specific assessments, and documenting results with precision.
• Formulate and communicate tailored treatment plans based on evaluation findings, engaging patients and their families in the goal-setting process to promote adherence and motivation.
• Implement and adapt treatment protocols in alignment with patient needs, utilizing current evidence-based practices to ensure optimal outcomes.
• Collaborate effectively with a multidisciplinary team to address patients' changing conditions, revising care plans as necessary and facilitating necessary referrals for additional medical or rehabilitation consultations.
• Ensure meticulous documentation of patient care activities, including clear initial evaluations, daily encounter notes, interim progress summaries, and discharge summaries, in compliance with healthcare regulations.
• Manage time and productivity efficiently, maintaining an accurate treatment schedule while being actively involved in departmental initiatives aimed at enhancing patient experience and access to care.
• Participate actively in departmental meetings, contributing to the advancement of team goals and adherence to regulatory standards.
• Engage in continuing education and mentorship roles, guiding junior staff and students to align with best practices and foster professional growth.
• Drive initiatives for patient access and service excellence, engaging with community outreach activities to promote awareness and education about physical therapy.

Patient Population / Client Focus

As a Physical Therapist at Active Physical Therapy, you will work primarily with a varied demographic of patients recovering from surgeries, injuries, and chronic conditions. Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution.
• Proof of graduation from a recognized school of Physical Therapy (High School Diploma or GED).
