Key Responsibilities\n

• Conduct thorough mental health assessments, considering how co-occurring medical conditions impact patients' psychological well-being.

• Develop and implement comprehensive treatment plans that address both psychiatric and medical concerns, tailoring evidence-based interventions to meet the unique needs of each patient.

• Facilitate individual and group therapy sessions, utilizing therapeutic modalities that reflect the current best practices in mental health treatment.

• Collaborate with medical professionals within our integrated care team to provide coordinated treatment and ensure the best possible outcomes for our patients.

• Provide crisis intervention and support during mental health emergencies, demonstrating the ability to calmly manage acute situations.

• Educate patients on the relationship between their mental health and physical well-being, helping them to manage their conditions and make informed decisions regarding their care.

• Maintain accurate and confidential patient documentation in compliance with legal and ethical standards, utilizing electronic health record systems to facilitate communication across the care team.

• Participate actively in team meetings and case conferences, contributing insights to interdisciplinary care planning and continuously improving the quality of service delivered.

Patient Population / Client Focus\n

Qualifications\n

• A Master's or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution is required.

• Current and valid licensure as an LCSW, LPCC, or LMFT in the state of California.

• Proficient in addressing the mental health needs of individuals with co-occurring medical conditions, with demonstrated experience in integrated care settings.

• Outstanding interpersonal and communication skills, with the ability to build trusting relationships with patients and collaborate effectively with healthcare colleagues.

• Experience in maintaining professional boundaries and adhering to ethical guidelines as well as ensuring patient confidentiality.

• Familiarity with electronic health record (EHR) systems, with the capability to navigate various computer applications relevant to patient care.

• Ability to work collaboratively as part of an interdisciplinary team.

Preferred Experience\n

• A minimum of 2 years of experience in a mental health counseling role, particularly within an integrated care setting.

Compensation and Benefits\n
