Key Responsibilities\n

• Conduct comprehensive assessments of the counseling needs for assigned clients, ensuring individualized treatment plans.

• Utilize DSM criteria to evaluate clients for substance abuse diagnosis and determine the appropriate level of care according to ASAM standards.

• Prepare and implement the substance abuse counseling components of client treatment plans, collaborating with interdisciplinary teams for holistic care.

• Develop and maintain discharge and continuum of care plans to facilitate patient transitions across different levels of care.

• Document all counseling sessions, assessments, referrals, and follow-ups in client clinical records diligently, ensuring adherence to confidentiality and legal standards.

• Manage a caseload effectively, providing necessary case management and recognizing moments for clinical charting and treatment planning.

• Coordinate support services for clients and their families, actively involving them in the therapeutic process and recording all relevant activities.

• Engage with referral sources such as legal representatives, schools, or employers to ensure a comprehensive understanding of each client’s unique situation.
