This full-time remote position focuses on providing emotional and psychosocial support to seniors within a Hospice Center setting. Work Environment\n

This position allows you to work remotely, enabling flexible scheduling while still connecting with clients in a meaningful way. The potential for a full-time work schedule from Monday to Friday, typically from 8:00 AM to 4:30 PM, provides structure, while occasional evening or weekend hours may be necessary to meet client needs or respond to urgent situations. As part of our commitment to continuous care, on-call availability may also be required in order to provide thorough support at all times.

Key Responsibilities\n

• Conduct comprehensive assessments of clients' psychosocial needs, evaluating emotional and social factors that influence their overall health.

• Collaborate closely with clients and family members to develop personalized care plans that reflect their specific goals, preferences, and life transitions.

• Provide one-on-one counseling services, helping seniors navigate challenges associated with aging, health issues, and family dynamics.

• Advocate strongly for clients' rights and their access to vital community resources, ensuring appropriate support services are readily available.

• Work collaboratively with an interdisciplinary team—including nurses, caregivers, and healthcare professionals—to coordinate comprehensive care delivery.

• Conduct regular check-ins, visiting clients in their homes or through virtual platforms to monitor their well-being and identify potential safety concerns.

• Facilitate family meetings to enhance communication and resolve conflicts surrounding each client's care.

• Educate clients and their families about community resources, including support groups and programs that promote social engagement and enhance quality of life.

• Maintain clear and accurate documentation of assessments, care plans, and client interactions to ensure continuity of care and compliance with organizational standards.

• Stay current with developments in social work practices, policies, and legislation, adhering to ethical standards and best practices in the field.

Your primary focus will be on seniors living in a hospice setting, many of whom may be grappling with complex emotional and physical health challenges related to end-of-life care. You will be instrumental in addressing their psychosocial needs and those of their families, advocating for their preferences and ensuring a respectful transition towards end-of-life care.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• A current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

• A minimum of two years of experience in a social work role, ideally in a geriatric care or hospice setting.

Compensation and Benefits\n
