Key Responsibilities

• Conduct thorough evaluations for patients, gathering detailed medical histories, performing assessments, and articulating treatment recommendations based on findings.
• Communicate evaluation results and devised treatment plans to patients and their families, ensuring their active involvement in goal-setting.
• Implement tailored treatment protocols effectively, demonstrating proficiency in adapting interventions based on each patient's unique presentation and responses.
• Collaborate with an interdisciplinary team of healthcare professionals to optimize patient care. Document any significant changes in a patient's condition that might necessitate adjustments to their treatment plan.
• Prepare comprehensive discharge plans and ensure timely follow-up care. Maintain meticulous documentation reflecting the patient's journey, including evaluations, treatment plans, progressive summaries, and discharge evaluations.
• Manage time effectively to meet productivity targets while maintaining an accurate schedule and being punctual to meetings.
• Engage in departmental initiatives aimed at excellence in patient care and community outreach, actively participating in marketing activities to promote our services.
• Pursue continuing education and professional development, serving as a mentor and clinical instructor to physical therapy students.

Patient Population / Client Focus

Qualifications
