Work Environment

This role is based entirely onsite in the patient's home, allowing you to create personalized therapy sessions in a comfortable and familiar environment. Key Responsibilities

• Conduct thorough evaluations of patients upon referral from physicians, establishing individualized treatment plans tailored to their specific needs.
• Implement treatment modalities and therapeutic exercises aimed at improving patients' mobility and overall function.
• Utilize evidence-based techniques to facilitate patient recovery and monitor progress while adjusting treatment strategies as necessary.
• Educate patients and their families on therapeutic exercises, safety protocols, and techniques to assist recovery outside of therapy sessions.
• Maintain meticulous documentation of patient assessments, treatment plans, and progress notes, ensuring compliance with organizational policies and legal requirements.
• Collaborate with interdisciplinary teams, including physicians and occupational therapists, to optimize patient care delivery and achieve favorable outcomes.
• Foster positive relationships with patients by demonstrating compassion and exceeding their expectations through excellent customer service.
• Meet or exceed productivity standards and business objectives related to patient care and utilization goals.

Patient Population / Client Focus

In this position, you will primarily treat a broad spectrum of patients, including orthopedic cases, pediatric patients, neurologic disorders, and sports-related injuries. You will be actively engaged with individuals requiring skilled therapeutic interventions to reclaim their mobility and improve their quality of life. A commitment to understanding each patient's unique challenges and recovery goals is paramount to ensuring successful interventions.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) from an accredited institution.
• Valid state license as a Doctor of Physical Therapy, issued by the Physical Therapy Examining Committee.
• A minimum of two (2) years of relevant clinical experience in physical therapy.
• Current CPR certification and a commitment to maintaining certifications and educational requirements.
• Exceptional verbal and written communication skills.
• Proficient in computer software relevant to patient documentation and treatment planning.
• Ability and desire to build strong, trusting relationships with patients and their families.
• A proactive approach to professional development and community engagement.

Preferred Experience

While a minimum of two years of experience is required, candidates with specialized training in areas such as sports medicine, pediatric therapy, or neurological rehabilitation will be viewed favorably. Experience in home health settings is also beneficial, as it provides a context for delivering care in patient-centric environments.

Compensation and Benefits
