Key Responsibilities\n

• Conduct comprehensive assessments of patients' psychosocial needs and develop personalized treatment plans that address their unique situations.

• Provide short-term counseling, case management, and crisis intervention services in alignment with best practices.

• Guide patients in securing benefits by assisting them with the completion of forms for entitlements, supplemental insurance, and other resources aimed at reducing reliance on emergency assistance programs.

• Support patient access to crucial prescription cost assistance programs through community resources and pharmaceutical company initiatives.

• Act as a patient advocate while liaising with community agencies to secure necessary services and resources.

• Maintain meticulous and accurate patient records, compiling reports and documentation to monitor treatment progress and outcomes.

• Facilitate transportation assistance for patients as needed to ensure they can attend appointments and receive care.

• Participate actively in clinical meetings and case conferences to foster a team-oriented approach and share insights on patient progress.

Patient Population / Client Focus\n

As a Licensed Mental Health Counselor at Resilience Psychiatry Solutions, you will work with a diverse patient population, including adolescents and adults facing issues such as anxiety, depression, trauma, and other mental health disorders.
