Key Responsibilities\n

• Conduct comprehensive assessments and ongoing evaluations to tailor treatment plans that meet the unique needs of each patient.

• Maintain regular communication with supervisors, direct care staff, family members, and relevant stakeholders concerning client progress and treatment adaptations.

• Prepare and present a tentative schedule to supervisors detailing appointments, home visits, and necessary observations for monitoring client development.

• Coordinate in-home visits for assessments, chart reviews, and staff interviews to gather necessary data, ensuring effective support for clients.

• Oversee the implementation of the therapeutic program plan, assuring it is executed effectively for optimal client outcomes.

• Participate actively in interdisciplinary team meetings and collaborate with colleagues to enhance treatment strategies.

• Document monthly and quarterly progress notes to track client developments and to inform treatment decisions.
\n
• Organize training sessions to ensure staff members are equipped with the competence required for effective program implementation and documentation.

• Provide follow-up support through interviews and observations to assist clients in achieving ongoing goals.

• Keep meticulous records that facilitate easy access to information for all parties involved in the client’s care.

• Act as a liaison to the public and the broader mental health community, advocating for and supporting developmental goals.

Qualifications\n
