Key Responsibilities\n

• Conduct thorough patient assessments to evaluate mental health diagnoses, employing tools such as the DSM (Diagnostic and Statistical Manual of Mental Disorders) to formulate an individualized care plan that accounts for cultural diversity and unique patient needs.

• Develop and implement treatment strategies and interventions while continuously monitoring patient progress and adjusting care plans as necessary to optimize outcomes.

• Maintain accurate and timely treatment records, reports, and clinical documentation using electronic health record systems, ensuring compliance with all regulatory standards and departmental protocols.

• Collaborate proactively with patients to create comprehensive treatment plans, integrating family involvement where appropriate to address their needs and goals.

• Leverage teamwork by actively engaging in multi-disciplinary consultations, anticipating patient safety concerns, and contributing to the overall effectiveness of the clinical team.

• Serve as a key liaison by coordinating referrals and establishing meaningful connections with other treatment providers and resources available to patients.

• Engage in the education and mentorship of students, interns, and volunteers, contributing to their professional development and fostering a supportive learning environment.

• Participate in clinical collaborations, conducting peer reviews and audits under the guidance of a clinical supervisor, to enhance the overall performance of the team.

Qualifications\n

• A Master’s degree in Social Work is required for this position.

• A current State License as a Licensed Clinical Social Worker (LCSW) is mandatory.

Compensation and Benefits\n
