This role offers a flexible work setting, primarily remote with occasional in-person meetings and client visits within the Edinburg community. Our working hours are structured from Monday to Friday, 8:00 AM to 4:30 PM, with some requirement for evening or weekend availability based on client needs. Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to identify the emotional and social needs of seniors, considering their unique life circumstances.

• Develop tailored care plans in conjunction with clients and their families, ensuring these plans align with their goals and preferences.

• Provide counseling and emotional support, helping seniors navigate life transitions such as relocation, loss, or health changes.

• Advocate on behalf of clients to ensure they have access to community resources, benefits, and support services that can help improve their lives.

• Perform regular home visits to assess living conditions and well-being, address potential safety and health concerns, and monitor the effectiveness of care plans.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Complete and active licensure as a Licensed Clinical Social Worker (LCSW) in Texas.

• A deep understanding of social work principles, particularly those relevant to aging issues and support systems available within the community.
