Work Environment

This Physical Therapist role operates onsite in a home care setting, offering the unique opportunity to engage directly with patients in their own environments. Our team-focused culture is designed to foster collaboration among healthcare providers, ensuring that patient care remains our utmost priority. The shifts for this position typically run from 9:00 A.M. to 5:00 P.M., allowing for a balanced work-life schedule.

Key Responsibilities

• Conduct comprehensive evaluations of patients, utilizing both subjective and objective measures based on their medical history and physician referrals.
• Design individualized treatment plans that incorporate best practices and clinical guidelines tailored to meet each patient's unique needs and goals.
• Implement therapeutic interventions using evidence-based physical therapy modalities to address various conditions affecting mobility and function.
• Provide education and training to patients and their caregivers, ensuring a clear understanding of treatment objectives and home exercises.
• Document all patient interactions and treatment progress meticulously in compliance with regulatory and facility guidelines.
• Facilitate communication with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to optimize patient care.
• Mentor and supervise therapy assistants and support staff in the delivery of high-quality care.
• Participate in quality improvement initiatives aimed at enhancing service delivery and patient satisfaction.
• Engage in ongoing professional development opportunities to stay current with advancements in physical therapy.
• Assist with administrative tasks, including monitoring department budgets, scheduling, and conducting utilization reviews when requested.

Patient Population / Client Focus

The ideal candidate will demonstrate proficiency in treating a diverse patient demographic, including neonates, children, adolescents, adults, and geriatric individuals. A strong emphasis is placed on delivering therapeutic services that cater to each of these populations, adapting treatment approaches based on varying levels of physical ability and understanding.

Qualifications

• A current state Physical Therapist license is essential.
• A Bachelor's degree or higher from an accredited physical therapy program is required.
• Demonstrated clinical experience in a healthcare environment is necessary, with familiarity in acute, outpatient, or home care settings being advantageous.
• Physical capability to perform tasks involving lifting, standing, and mobility exercises with patients.
• Effective communication skills to engage with patients, families, and colleagues alike.

Preferred Experience
