Key Responsibilities\n

As a Licensed Clinical Social Worker at Greater City Family Counseling, your primary responsibilities will include:

• Conducting comprehensive assessments and evaluations of patients' mental health statuses using evidence-based tools, such as the Diagnostic and Statistical Manual of Mental Disorders (DSM), to facilitate accurate diagnoses and treatment planning.

• Developing and maintaining individual treatment plans that are culturally competent and tailored to the specific needs and goals of each patient.

• Monitoring patient progress continuously, providing appropriate interventions, and adjusting treatment objectives as necessary to improve outcomes.

• Documenting all clinical information, treatment records, and reports in a timely and accurate manner, adhering to regulatory requirements and internal protocols.

• Collaborating with multidisciplinary team members in creating holistic care plans that encompass both mental and physical health considerations for comprehensive treatment.

• Coordinating referrals and acting as a liaison between patients and other treatment providers, ensuring seamless connectivity for patient support.

Qualifications\n
