Work Environment

This position is onsite within our service area, primarily focusing on home care settings. Our Physical Therapists utilize a patient-centric approach while emphasizing communication and teamwork. Key Responsibilities

• Conduct comprehensive evaluations of patients, including gathering relevant medical histories and performing precise functional assessments to inform treatment decisions.
• Communicate evaluation findings and proposed treatment plans effectively to patients and their families, encouraging active participation in the goal-setting process.
• Implement tailored treatment protocols aligned with each patient's individual needs and specific health conditions, continually adapting approaches as required.
• Engage in multidisciplinary consultations with physicians and other healthcare professionals to address changes in patient health and modify care plans accordingly.
• Ensure meticulous discharge planning by documenting all patient care activities promptly, including evaluations, interim progress notes, and discharge summaries that reflect the patient's journey through therapy.
• Manage your schedule efficiently to meet productivity expectations while contributing positively to team goals and participating in marketing initiatives to enhance community outreach.
• Pursue ongoing professional development through continuing education opportunities and serve as a clinical instructor for students in physical therapy programs.
• Promote a culture of exceptional service by providing a responsive and adaptable patient experience focused on accessibility and satisfaction.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) from an accredited institution.
• A valid physical therapy license in the state of New Jersey.
• Current American Heart Association Basic Life Support (BLS) certification is required for all healthcare providers.
• Strong interpersonal skills with a focus on patient engagement and collaborative care.

Preferred Experience

• A minimum of two years of relevant experience in a clinical setting is preferred but not mandatory; new graduates are welcome to apply.

Compensation and Benefits
