Work Environment

This position requires on-site work where you will engage directly with patients in their homes across Punta Gorda and surrounding areas. Key Responsibilities

• Conduct thorough evaluations to assess patients' functional limitations and develop comprehensive, individualized treatment plans.
• Implement and manage therapeutic interventions, ensuring that patients receive the highest quality of care to meet their rehabilitation goals.
• Consult with healthcare providers to ensure a seamless continuum of care, including discussing changes to treatment protocols with physicians when necessary.
• Maintain accurate documentation of treatments and patient progress, adhering to all relevant laws and regulations while submitting necessary billing information.
• Educate patients and their families about their conditions, treatment plans, and home exercise programs to facilitate ongoing recovery post-therapy.
• Supervise and mentor support staff including Physical Therapy Assistants (PTAs) and aides to optimize resource management and therapeutic delivery.
• Engage in continuous learning through professional development opportunities, departmental meetings, and in-service training.
• Participate in community outreach programs aimed at promoting health awareness and preventative care.

Patient Population / Client Focus

You will primarily work with patients experiencing a range of orthopedic issues, including but not limited to post-surgical rehabilitation, joint replacements, fractures, and various musculoskeletal disorders. Your expertise will assist patients of diverse ages and backgrounds, ensuring culturally competent care that addresses their unique needs and preferences.

Qualifications

Candidates must possess at least a Bachelor's Degree in Physical Therapy from an accredited institution. A Doctorate in Physical Therapy is preferred. You should demonstrate strong verbal and written communication skills, the ability to work collaboratively with interdisciplinary teams, and a commitment to adhering to ethical and regulatory standards in patient care.

Preferred Experience

While all levels of experience will be considered, preference will be given to candidates with prior experience in home health care or orthopedic rehabilitation settings. Familiarity with electronic health record (EHR) systems and knowledge of the latest therapeutic techniques will be advantageous.

Compensation and Benefits

We offer a competitive salary range of USD 100,000 to 120,000 yearly, commensurate with experience and credentials.
