Key Responsibilities\n

• Conduct comprehensive psychosocial assessments using interviews, clinical observations, and medical record reviews to evaluate the emotional, social, and environmental factors affecting patient recovery and comfort.

• Provide individual and family counseling to address adjustment issues, grief, and emotional health, enabling patients and their families to navigate the complex emotions associated with end-of-life care.

• Collaborate closely with the interdisciplinary team—including nurses, physicians, and occupational therapists—to develop tailored treatment plans that incorporate each patient's unique psychosocial needs.

• Assist patients in accessing essential community resources, including support groups, bereavement counseling, and practical assistance programs, to ease their transition during difficult times.

• Educate patients and families regarding available community resources and post-hospice services, ensuring they have the necessary tools for ongoing support after care concludes.

• Participate in discharge planning efforts and facilitate smooth transitions to home or alternative care settings, ensuring continuity of care.

Patient Population / Client Focus\n

The clinical social worker will primarily serve patients who are in the late stages of terminal illnesses, dealing with various conditions such as cancer, dementia, and heart disease. Your interactions will involve understanding complex emotional responses from both patients and their families, which often include fear, denial, and grief. Qualifications\n

• Master's degree in Social Work from an accredited institution.

Compensation and Benefits\n
