Work Environment

As a Physical Therapist at Ability Rehab PT, you will work onsite in the home setting, providing therapy services directly to patients in their residences. Key Responsibilities

• Conduct comprehensive evaluations, considering patients' medical histories and performing specific assessments to formulate individualized treatment plans.
• Engage with patients and their families, explaining evaluation findings and collaboratively establishing rehabilitation goals.
• Implement evidence-based treatment protocols tailored to the patient's condition, ensuring progressive rehabilitation within the home environment.
• Maintain communication with a multidisciplinary healthcare team to address any changes in patient conditions, modifying care plans as necessary.
• Document all patient care activities accurately, including daily progress notes and discharge planning, to provide a clear continuum of care.
• Enhance overall productivity by effectively managing time and scheduling follow-up visits while participating in community outreach initiatives.
• Promote a superior patient experience by adapting service delivery to meet patient expectations and care access requirements.

Patient Population / Client Focus

You will work primarily with patients recovering from orthopedic surgeries, neurological disorders, and chronic pain conditions. Our focus is on a patient-centered approach, ensuring that treatment plans are not only effective but also accommodating to the day-to-day challenges faced by individuals in their home environments.

Qualifications

• Doctorate of Physical Therapy (DPT) from an accredited institution.
• Current state licensure as a Doctor of Physical Therapy (DPT) by the Virginia Physical Therapy Examining Committee.
• A minimum of two years of experience as a practicing Physical Therapist.
• Current CPR/AED certification required.

Preferred Experience

While a minimum of two years' experience is required, candidates with experience in home health physical therapy or a home care setting are preferred. Familiarity with electronic health record systems and experience with patient-centered therapy models will be an asset.

Compensation and Benefits

This position offers a competitive salary range of USD 100,000 to 120,000 annually, commensurate with experience.
