Work Environment\n

This role is designed for remote work, allowing our Licensed Clinical Social Workers to effectively manage their schedules while delivering critical mental health services. You will coordinate with a multi-disciplinary team, including Reentry Social Workers and clinical supervisors, to facilitate discharge planning and access to treatment for our clients. Key Responsibilities\n

• Collaborate closely with Reentry Social Workers to execute effective discharge planning for clients transitioning from institutional settings.

• Prepare and submit required statistical reports to track the effectiveness of treatment protocols.
\n
• Establish and maintain strong working relationships with the Department of Correction to secure proper housing and therapeutic dispositions for patients.
\n
• Develop and facilitate structured group treatments within mental health observation units and other designated areas.
\n
• Maintain meticulous documentation of patient interactions, therapies administered, and treatment progress in accordance with regulatory and organizational standards.
\n
• Engage in weekly clinical supervision sessions with the Clinical Supervisor to enhance professional skills and patient care strategies.
\n
• Participate actively in clinical rounds, case conferences, and treatment planning meetings to ensure comprehensive care coordination for mental health patients.
\n
• Conduct mental health evaluations, crisis interventions, comprehensive assessments, and follow-up care effectively and empathetically.
\n
• Determine appropriate treatment dispositions and coordinate care plans based on evaluations, ensuring patients receive appropriate mental health services.

This position focuses primarily on children and adolescents experiencing mental health challenges. You will be instrumental in supporting these young individuals and their families through difficult times, facilitating access to therapeutic resources, and building resilience through structured interventions. Qualifications\n

• A Master’s or Doctorate degree in Social Work, Counseling, Psychology, or a related mental health discipline.
