Key Responsibilities

• Conduct thorough evaluations of patients, including the review of medical histories and the implementation of specific assessments to determine individualized treatment recommendations.
• Clearly communicate evaluation findings and proposed treatment plans to patients and their families, actively involving them in goal-setting processes.
• Demonstrate proficiency in the execution of treatment protocols that respond to the unique needs of each patient while adapting to the evolving healthcare environment.
• Collaborate with fellow healthcare professionals to ensure comprehensive care, recognizing any changes in patient conditions that may warrant adjustments in the treatment plan.
• Ensure meticulous documentation of patient care activities, including initial evaluations, daily notes, progress summaries, and discharge plans, to foster continuity of care within our evolving system.
• Manage time efficiently and meet productivity targets, maintaining a carefully organized schedule and punctual attendance to meetings.
• Engage actively in hospital and community marketing initiatives to promote our services and enhance patient access to care.
• Contribute to departmental objectives by taking initiative in pursuing continuing education for professional growth and serving as a mentor for students in clinical placements.

Patient Population / Client Focus

Our rehabilitation facility serves a diverse patient population, addressing a variety of orthopedic, neurological, and post-surgical conditions. Qualifications

• A Bachelor's degree in Physical Therapy is required, while a Doctorate in Physical Therapy is preferred.
• Current state licensure to practice as a Physical Therapist in New York.
• Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.
• Strong interpersonal and communication skills, necessary for effective interaction with patients, families, and team members.

Preferred Experience

Compensation and Benefits
