Key Responsibilities\n

• Assist primary care providers in identifying, treating, and managing mental health and psychosocial issues, becoming a key ally in their holistic approach to patient care.

• Deliver brief, focused interventions, catering specifically to individuals requiring mental health services.

• Utilize your clinical judgment to determine whether patients can be effectively treated in primary care or need referral to specialized mental health services.

• Follow up with patients to monitor their progress, adjust treatment plans as necessary, and help develop relapse prevention strategies to promote stable functioning.

• Coordinate with various healthcare providers to ensure seamless delivery of health services across settings, respecting patient consent for external referrals.

• Maintain accurate documentation of patient progress and treatment interventions, keeping the primary care team informed about patient needs and developments.

• Empower patients by providing self-management techniques and educational resources that enhance their active participation in their recovery journey.

• Consult with clinic management and team members to enhance the efficacy of behavioral services and contribute to program evaluation efforts.
