Work Environment

This position involves working onsite, primarily in a home care setting. Key Responsibilities

• Conduct thorough evaluations to identify patient needs, goals, and limitations, and subsequently plan suitable treatment protocols.
• Implement cost-effective and evidence-based treatment strategies to improve patients' mobility and enhance their physical capabilities.
• Collaborate with patients to design personalized care plans that incorporate various treatment techniques, exercises, and wellness programs.
• Motivate and guide patients during therapy sessions, employing encouragement and support to foster a positive mindset.
• Monitor patients' progress over time, adjusting treatment plans as necessary to meet individual goals effectively.
• Provide education on lifestyle modifications and ergonomic strategies to promote long-term wellness and prevent future injuries.
• Work alongside other healthcare professionals to integrate patient care, ensuring comprehensive support throughout the rehabilitation process.
• Document patient treatment plans, progress, and outcomes accurately, maintaining communication with patients and referral sources.

Patient Population / Client Focus

The ideal candidate will work primarily with patients recovering from orthopedic injuries, surgeries, and chronic musculoskeletal conditions. Your role will involve addressing a spectrum of challenges, from post-operative rehabilitation to injury prevention strategies, ensuring that each patient receives the individualized care they need to restore function and independence.

Qualifications

• An active State license as a Physical Therapist.
• A Bachelor's Degree in Physical Therapy is required; a Master's Degree or Doctorate from an accredited program is preferred.
• Strong interpersonal and communication skills with a demonstrated ability to engage and educate patients effectively.

Preferred Experience

• At least 2 years of clinical experience in orthopedic rehabilitation, particularly with patients experiencing musculoskeletal conditions.

Compensation and Benefits
