Swift Therapy Center is actively seeking a skilled and compassionate Physical Therapist to join our dedicated team in Naples, Florida. This full-time position focuses on providing high-quality, patient-centered care in a home care environment. Work Environment

This position is based onsite within the home care setting, allowing therapists to engage with patients in their own environments. The work schedule runs from 8:00 am to 5:00 pm, Monday through Friday, ensuring a balance between professional responsibilities and personal time. Key Responsibilities

• Examine, evaluate, and develop tailored treatment plans for patients experiencing functional limitations and disabilities.
• Utilize assessment tools and relevant patient history to document findings and create comprehensive recommendations for ongoing care.
• Implement personalized treatment protocols based on evaluation results and the patient's evolving condition.
• Collaborate with multidisciplinary teams by consulting with physicians and other healthcare professionals to optimize patient outcomes.
• Document patient progress and modify treatment plans as needed, ensuring compliance with local, state, and federal regulations.
• Plan and coordinate discharge processes, providing clear communication on follow-up care and patient needs.
• Attend facility meetings and team initiatives aimed at improving patient access and care quality.

Patient Population / Client Focus

The primary focus of this role is on outpatient orthopedic patients who require physical therapy to recover from injuries or manage chronic conditions. You will work closely with individuals of varying ages and backgrounds, developing customized treatment approaches that respect their unique needs and lifestyle, while fostering an empowering and supportive treatment atmosphere.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited educational institution is required.
• A valid physical therapy license in Florida, or eligibility to obtain one.

Preferred Experience

• A minimum of two years' experience in an outpatient orthopedic setting, demonstrating a strong clinical skill set and familiarity with rehabilitation practices.
• Current CPR certification to ensure patient safety and emergency preparedness.

Compensation and Benefits
