Key Responsibilities\n

• Conduct thorough and compassionate assessments of clients' social, emotional, and environmental challenges to create a holistic understanding of their needs.

• Develop and implement personalized service plans that align with clients' specific goals and monitor their progress toward achieving those objectives.

• Provide individual and family counseling, utilizing evidence-based practices to facilitate healing and personal growth.

• Collaborate with clients to connect them with local community resources, ensuring they receive comprehensive support that addresses their multifaceted needs.

• Offer immediate crisis intervention, conducting risk assessments and developing safety plans to protect clients in distress.

• Engage with community agencies and organizations to coordinate comprehensive services and advocate for clients’ rights and needs.

• Participate in community outreach initiatives designed to elevate awareness and accessibility of our agency's services.

• Attend and contribute to case conferences, team meetings, and ongoing training sessions to remain current in best practices and enhance service delivery.

Patient Population / Client Focus\n

In your capacity as an LCSW at Community Outreach Network, you will primarily serve individuals and families grappling with significant hardships, including terminal illness within the hospice setting. This vulnerable population requires your expertise not just in clinical therapy, but also in navigating complex emotional, social, and practical issues associated with end-of-life care. Qualifications\n

• A Master's degree in Social Work from an accredited program is required.

• Possession of an active state license as a Licensed Clinical Social Worker (LCSW) is mandatory.

Compensation and Benefits\n
