Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for individuals, families, and groups to determine their mental health needs and develop tailored treatment plans.

• Implement appropriate graduate-level clinical social work or counseling interventions aimed at improving patients' mental health outcomes.

• Engage in ongoing communication with other healthcare professionals within the interdisciplinary team to ensure cohesive care and collaboration.

• Utilize documentation and electronic health record systems to maintain accurate records of patient interactions, treatment progress, and recommendations.

• Facilitate case management for assigned patients, coordinating services with community resources and healthcare providers to streamline their path to recovery.

• Deliver Cognitive Behavioral Therapy (CBT) and other evidence-based treatment modalities to help clients achieve their therapeutic goals and implement effective strategies for their well-being.

• Complete and submit documentation in a timely and ethical manner, complying with departmental standards and best practices.

Qualifications\n

• Licensure as a Licensed Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC), or Licensed Clinical Social Worker (LCSW) in the State of California is required.
