Work Environment\n

This position is fully remote, providing you with the flexibility to manage your schedule while ensuring comprehensive care delivery to clients. You will be part of a community health clinic focused on in-home mental health services, where you will engage with diverse populations. Key Responsibilities\n

• Conduct thorough assessments of clients to evaluate their mental health needs and establish appropriate treatment plans tailored to individual circumstances.

• Maintain clear and consistent communication with supervisors, direct care staff, and families to monitor and discuss client progress effectively.

• Develop and share a tentative schedule of upcoming appointments, home visits, and observations to provide transparency in your workflow.
\n
• Coordinate and conduct home visits, which are essential for observations and evaluations, ensuring a focused and tailored approach to client care.
\n
• Oversee the implementation of treatment plans, ensuring all interventions are effectively executed.
\n
• Participate actively in Interdisciplinary Team Meetings, contributing to discussions that enhance client care and program development.
\n
• Document client progress through monthly and quarterly notes, keeping all records accurate and readily available.
\n
• Facilitate training sessions for staff to ensure they are well-versed in the implementation of treatment protocols and documentation requirements.

• Provide ongoing support through follow-up interviews and home visits, continually assessing and addressing clients' evolving needs.
\n
• Serve as a liaison with the community and public mental health resources to promote the development of accessible mental health services.

Patient Population / Client Focus\n

Your work will focus on servicing a varied client base, including adults, youth, couples, and families facing a range of mental health challenges. You will utilize evidence-based practices to support individuals experiencing trauma, substance misuse, anxiety, depression, and family dynamics.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is mandatory.

• Possession of an active and unrestricted LPC license in Pennsylvania is required.

• A minimum of two years of counseling experience in a clinical work setting.
\n
• Expertise in family therapy, trauma-focused therapy, or substance abuse counseling is preferred.

• A robust understanding of ethical and legal standards in mental health practice.
\n
• Exceptional interpersonal and communication skills to build rapport with clients from diverse backgrounds.

• Proven ability to work collaboratively within a healthcare team.
\n
• Familiarity with electronic medical records (EMR) systems and related technology is advantageous.
